just copy all files of 5.4.2 to cover 5.4.1?

You'd need to copy all of the files over and then bring up the altertable-5.4.2-5.4.3.php file in your web browser. Whenever you are going to upgrade you will want to take a look at the UPGRADING file to see if there are any specific instructions for upgrading to the latest version.
