We had a user, Shannon L. Wolf, who was once the moderator for our Stomach Issues site. You can see a post of hers at the bottom of this page of discussion -

When she left the Stomach Issues site, we removed her moderation flag. However it still shows a "M" by her name. I have tried re-activating and deactivating her in the user screen, and also in the moderation screen. I even tried activating her on a different area and then deactivating her. No matter what I do, that M flag remains on her page. When I go in to edit her it definitely says Member Status: User and NOT Moderator.

Any ideas how she got stuck in this state?

Yes this is a bug due to the last change that allows users to select what group graphic displays next to the name.
