These are Apple II VGA boards designed by Briel Computers. I made up a couple of these cards for myself and the GitHub project is excellent. Notice the 2 photos showing the output on a VGA monitor from my Apple iie. Should work with Apple II, II+ or IIe. Even supports double Hi-res mode on the Apple iie. The pcb has a gold ENIG finish. If you know how to solder you can build one of these very quickly and inexpensively and have great looking graphics from your Apple II. Just uses a handful of parts: resistors, 3 ic's, a few capacitors, VGA socket and a cheap $4 Pi Pico Microcontroller board. No modifications to your Apple are needed. Just plug into the socket and turn on your computer. You can even use your old Composite video output at the same time but why look at a blurry screen ever again?
