In my profile, I have "yes" selected for:
"By default should anything added to your Watch Lists be emailed to you?"
If I elect to add an existing topic to my watch list, it correctly appears in my watched topics list for receiving emails.
However, if I elect to watch a topic right as I start it, in my profile, it does not show as set to receive email notifications. I manually have to do that.
Designed or bug?
