I have also never manually approved members and never will.
We also use the same system as jgeoff and have never had any real issues.
Any obvious spammers are unlikely to approve their email address.
Any genuine person who is going to manually spam is also going to look genuine and you will probably approve them.
If you are concerned then simply restrict new members until they reach say 10 posts or manually approve their first couple of posts.
