Okay I found it finally.
stylepreview.tmpl calls:
Code
<img src="{$config['BASE_URL']}/images/blank.gif" height="5" width="50" />
Since the zip file does not include that file in that folder it should be:
Code
<img src="{$config['BASE_URL']}/images/general/default/blank.gif" height="5" width="50" />
