Those are kinda generic errors. Tough to tell what might be happening.
Do you get the same errors if you try and run it from the browser? Or do you get a blank page?
You might need to adjust the require line in the script to make sure it's finding the threads stuff ok.
Look for this line:
require ("../../main.inc.php");
