I'm sure this is not an issue but I cant find the answer anywhere.
The shareaholic strip shows up in some forums and not all ?
What's the criteria for it to show and can it be modified ?

Well it only shows in topic view. Not forum summary view.
But to answer your question.
I think shareaholic states that the topic must be accessible.

If the guest user can see the forum the shareaholic bar will display; if a guest user cannot see the forum the shareaholic bar will not display.

I've added the code to the html includes but it doesn't show when i do show page source.
My index page is my portal page. They have not replied yet.

It won't verify if you're using an index file to redirect to the ubb; you'll need to create an index.html/index.php document with just their code and then verify it, then change it back. They know about this bug as I reported it years ago.
