Atari part number C018187, model DV9319A. The label specifies 120V AC input and 11.5V DC, 1.95A output. Both attached cables and the original US two-prong plug are included.

The adapter shows cosmetic wear from age and previous use, including surface scratches, scuffs, and label wear. The cables and connector are present as shown.

Tested for power. Output voltage accuracy and extended operation under load were not independently verified.
