This is an LSI Logic Ultra320 SCSI Single-Channel PCI-X Host Bus Adapter, model LSI21320-IS (Revision A00). The green circuit board, mounting bracket, and internal/external pins are structurally in excellent condition with clean contact teeth as seen in the photos. The attached two-pin header wire is intact, and the rear soldering points show no signs of corrosion or blown capacitors.
