1) You have not completed or submitted the email settings. Go to control panel, primary settings, email settings and be sure you have filled in all boxes, especially the choice between html and text. Then submit, even if you do not change anything.
2) Once registered, names stay in the public_names file and cannot be reused. Go to Maintenance and Rebuild Member Stats to clear the deleted names.
