My problem was the URL path. I had the absolute path instead of the URL path. Under Control Panel » Feature Settings >> Atachments.

I have set the permissions to allow images to be inserted into posts and it is working.

However, the image does not show up. If you look at the source code that is getting generated you can see the end ">" is not being inserted.

Where is the php code for that function?
