Basically, an ad island is an island that is displayed between the first and second post in a thread. What you'd do is setup a custom island. In there you'd put in whatever ad you want, probably a horizontal/landscape type ad.
Then you can go into the forum settings and specify that ad in the "Custom Island Insert" section for the forum you want it to display in.
