Years ago when I had a thriving UBB community I had moderator agreements with all kinds of stuff in it to protect both the site owner and moderators. Also in this agreement I had some insights and rules on how to perform the moderator duties. I'm pretty busy with an overhaul on a camera security system at the moment but will make some time to find that and upload it for you to review and all that. It should give you some ideas.
