what would cause 500 errors in profiles/prefrences/registration, yet nothing else is affected?

I had attempted a custom image/title hack
this one in perticular: http://www.turbanator.freeserve.co.uk/IconHack.txt

and now get 500 errors whenever (I assume) a profile needs to be accessed?

edit: I'm asking this here because I replaced the hack with backup files and the problem remains

What UBB version is this for and could you please provide a direct link to your Ultimate.cgi?
