Where do you insert the html code? Tried numerous ways the picture does not show up, I unhide it in the portal and it still will not show up...?

The html goes in between the $body = <<<EOF and EOF; lines. So if you're trying to put a picture in place you'd do something like:

$body = <<<EOF
<img src="image_url" />
EOF;
