How do I set this up? Group image or are there also other ways?

They are two separate items.

The ubb 10 years is a group image.
So you would add a group and upload a image for that group.
Then for it to appear. First you would need to place that member in that group.
Then the member can edit their profile to elect if they wish to display it.

The ubbhelper is a custom title.
So you would need to manually upload the file on your site.
Then edit the user in the control panel.
And add a html tag in the custom title field like:
