Could you provide a link to your site? What version are you running? Are you running your site through a Content Delivery Network (CDN)? Does your webhost cache any on site resources? Do you have a .htaccess or httpd.conf rule that instructs browsers to cache images? such as: <FilesMatch "\.(flv|gif|jpg|jpeg|png|ico)$">

Also you can check Tools & Information-Permission checks See if images/forumimages/default (OK) if not you have a file.folder permission issue.

Replace Forum Image: 1) Click the "Browse" button next to the image you want to replace. 2) Browse your local files, and upload your new forum image. 3) Click the "Update/Remove Forum Images" button at the bottom of the page after uploading the image you want to replace.
The page will refresh and the new image will be displayed.
** If you are uploading an image with the same file name as the one you are placing, you may need to clear your browser's local cache.
if the above instruction does not work for you; instead of replacing images, delete the unwanted image and add a new one.
Delete Forum Image: 1) Put a check-mark in the "Delete" checkbox next to the unwanted image. 2) Click the "Update/Remove Forum Images" button at the bottom
Add Forum Image: 1) Click "Add New Forum Image" at the bottom of the page. 2) Browse your local files, and upload your new forum image. 3) Click the "Add New Forum Picture" button.

Okay I am confused. It appears that you are using a localhost install since the ip is 127.0.0.1 How can you have a windows server and use Apache.. Windows uses IIS. Are you using a local windows machine and have installed wamp,xampp,etc? If that is true, It can be a little tricky resolving absolute and relative paths.
