Custom Islands

Could someone shed some light on the use for this? It looks like I can put custom things inside them. What I would like to do is query our photo gallery and display a pic in there. However I am not sure how custom island works or even how it might not work for what I am doing.

Can you put an iframe in there or something?
Or can I just write some plain html in there like <img src="foo.jpg" alt="random photo" height="100" width="100" /> and expect it to render that for me or are there specific guidelines?How is it done?
